Community HealthChoices and ACT 150 Programs

 

Liberty Community Connections is an approved provider for the following Home and Community Based Waiver Programs and ACT 150 Programs at OLTL (Pennsylvania Office of Long Term Living):

 

• Community HealthChoices Waiver

• OBRA Waiver

 

These are programs that provide long-term care services and supports to individuals at risk for institutionalization but who want to remain living in their home and/or in the community.
